*NOTE: Please ENTER A SEPARATE FORM FOR EACH PERSON you wish to purchase a shirt for (so that we can keep sizing straight).   Please note that all sales are final.  Be sure that you double check your sizes and quantities before submitting your order form. Once your order form is submitted, you will receive a confirmation email from Google Forms. If you do not receive this email, your order has not been accounted for.  The total cost of your order will be charged by FootNotes to your credit card on file.
